Congratulations on your success in the 2024 SEA Examination. Welcome to the Fyzabad Secondary School Family.

UPDATE: **Please be advised Orientation will now be held on Monday 2nd September 2024

All students along with a parent/guardian are required to visit Fyzabad Secondary School to complete the registration process on Wednesday 3rd July, 2024 9AM to 12 noon

Document Guidelines

  • Students Names MUST be the same as indicated on the student's SEA Slip.

  • Birth Certificate Pin, please enter student's TEN (10)-digit pin number in the appropriate field.

  • Mother/Father's information on registration form and the child's Birth Certificate/Affidavit must be the same.

  • Parent/Guardian's ID may be: National Identification Card, Driver's Permit or Passport.

  • A Guardian can be either Legal, i.e., by custody court order, or assigned i.e., holder of a custody letter signed by a Justice of the Peace. This proof of guardianship should be uploaded when you see the prompt; 'Upload supporting Documents'

  • Students must register before a transfer application can be submitted.

  • Transfers are approved at the Ministry of Education (MOE) level ONLY.

  • At the end of the registration form, you 'MUST' click the 'SUBMIT' button to complete the process. Please wait until you see the confirmation page before you close the browser window.

  • N.B.: All information submitted via our online forms is automatically sent to us at Fyzabad Secondary School (FSS). At the same time, you will receive a copy of your responses via the email address you provided at the start of each form.

  • Parents/Guardians Are required to attend a registration session at Fyzabad Secondary School on Wednesday 3rd July 2024 from 9am to 12 noon.

  • An orientation session is scheduled for Friday, August 30th, 2024, at Fyzabad Secondary School, starting  at 9:00 AM. All students are required to attend in their full school uniform.

Fyzabad Secondary School is the owner of the online registration form. As such, only authorized students are eligible to register. Please note that you only need to submit the Online Form ONCE.

Required Registration Documents

Original and Scanned Copies of the following Documents are required for the registration process

​

  1. SEA Admission Slip

  2. Birth Certificate with supporting affidavit where applicable 

  3. Student Immunization Card.(Please Scan all pages)

  4. Parent/Guardian's ID front/ID rear  

  5. Recent Photo of Student (taken in front of a plain background, head and shoulders only). Student should be well groomed and preferably wearing a white round necked T-Shirt, no hats allowed

  6. Proof of guardianship (court order or document notarized by a JP)

  7. Student Permit (non-nationals only)​

On Wednesday 3rd July 2024:

1. The following items will be available for purchase at the school:

 2. TextBooks available through the MOE Textbook Rental Program will be distributed.

 3. Members of the P.T.A. executive will be present to meet with parents/guardians. They will manage the following activities:

        a. Collection of annual P.T.A contribution ($100.00)

        b. Sale of used textbooks
